The incidence of distant metastases in head and neck cancer especially in the salivary glands is relatively low compared with other malignancies. However, the presence of distant metastases announces a poor prognosis in head and neck, with a median survival of 4.3 to 7.3 months. Patients with malignant salivary glands should have an x-ray or CT scan of the chest at their initial assessment to exclude the possibility of distant metastases. The likelihood of developing distant metastases is associated with high-grade tumors such as adenoid cystic carcinoma, salivary duct carcinoma, mucoepidermoid carcinoma high grade tumors located in the submandibular gland, the tongue and posterior pharyngeal tumors. A lower risk of developing distant metastases is known to all other histological entities of salivary gland tumors. However, all patients who have a malignant tumor histologically confirmed salivary gland should be followed throughout.
